NDIS Support Items: Group support – centre-based or individual support – centre based

Support Ratio: 1:1, 1:2 or 1:3

• Core Supports – Assistance with Social and Community Participation

Activity fees vary (refer to activity planner)

Transport fee charge rate $1.00/km

An intake process needs to be completed prior to booking in for this group. This requires participants to have returned a signed service agreement, fee schedule, and new client intake forms.
