Catering for complex needs.
Horizons is a group support service which supports individuals with more complex needs. We have a purpose built space that is safe and welcoming where your loved one can explore, discover and grow.
The objective of this service is to facilitate access to a broad range of community-based activities with the right level of support.
Activities include:
- Art and craft
- Outings and excursions
- Social and emotional skills development
- Experienced and supportive staff
NDIS Support Items: Group support – centre based complex / Individual support – centre based
Support Ratio: 1:2 or 1:1
- Core Supports – Assistance with Social and Community Participation
- Capacity Building – Increased Social and Community Participation
Activity Fee: From $5 – invoiced separately. Refer to the activity planner.
Booking Requirements
An intake process needs to be completed prior to booking in for this group. This requires participants to have returned a signed service agreement, fee schedule, and new client intake forms.
